This includes any person or entity that suffered property damage and/or paid to repair property damage caused by the failure of an Affected Tank. Affected tanks include those stamped with model #3464 or #3412 with a manufacturing date between Jan. 1, 2011 and Dec. 31, 2011.
Estimated Amount: Up to $300/tank, up to $4000 in property damage reimbursement
Proof of Purchase: A minimum of proof of ownership of a Vortens toilet tank must be provided. Property damage claims must be supported by proof, including receipts, of the damage.
